Passports can be altered in the following circumstances:
Name Change
Profession Change
Change the status from Middle East to All Country
Amendments
Amendments, Inclusion and Deletion of Endorsements of a Passport for Name changes/ Profession changes
1. Documents Required
Name Change:
- Request Letter
- Duly filled Application, Form O (IM 37) Download Here
- Original passport.
- Passport copy of the same.
- Marriage Certificate. (Sinhala / Tami Original with a copy), if the surname of the husband should be included in the passport with a written request to change the name in the Passport after marriage.
- Sri Lanka National Identity Card copy
- Birth Certificate of the person concerned.
Profession Change:
- Request a letter from the person concerned to change the Profession in the Passport
- Service Letter mentioning the new designation on it.
- Duly filled Application Form Form O (IM 37) Download Here
- Original Passport
- Passport copy of the same.
- Educational, Higher Educational, and Professional qualifications to prove the new profession.
2. Fees
Per Alteration/ Endorsement : QAR 45/-
3. Additional Notes
THE CHANGE WILL BE ENDORSED ONLY AFTER THE APPROVAL OF THE CONTROLLER GENERAL IMMIGRATION & EMIGRATION COLOMBO SUBJECT TO THE SUBMISSION OF THE RELEVANT DOCUMENT.

PLEASE NOTE THAT THE APPLICANT SHOULD PHYSICALLY BE PRESENT TO SUBMIT THE APPLICATION AND FOR COLLECTION AS WELL.
