Documents to be submitted by the Next of Kin (NoK)

If the Next of Kin (NoK) is in Sri Lanka, the required documents should be submitted to the Consular Affairs Division of the Ministry of Foreign Affairs or to the relevant Divisional Secretariat and if the NoK is in Qatar, the required documents should be submitted to the Consular Division of the Embassy.

The required documents are as follows:

  • Details of the deceased (Passport Copy, National Identity Card and Birth Certificate)
  • Affidavit by the legal heir consenting to either repatriation or burial
  • Marriage Certificate, Birth Certificate of the applicant/ complainant to establish relationship if deceased was married. If not, parents of the deceased should submit the documents
  • Contact details and name of the foreign employer/agent
  • Details of the consignee who will be receiving the human remains in Sri Lanka
  • Additional documents will be required for any difference in name

 

Documents to be submitted by the Employer/Sponsor

The following documents should be submitted by the Employer/Sponsor to the Embassy

The required documents are as follows:

  • Death Notification
  • Death Certificate (Download here)
  • Embalming Certificate
  • Police Report (if necessary)
  • Body Transfer Certificate or Burial Notice
  • Original Passport of the deceased
  • Copy of cancelled Visa (QID) of the deceased
  • Other relevant reports and letters issued by the Hospital / Court / Police related to the death (if applicable)
  • Duly filled application form of the employer stating the details related to his salary dues, Gratuity and Compensation etc. (Download Here)
  • Airway Bill

Note: The No Objection Certificate (NOC) from the Embassy can only be issued upon receipt of all the required documents from the Next of Kin (NoK) and the Employer/Sponsor as listed above.
